By submitting this form, you acknowledge and agree on behalf of your school or service (Organisation) that the Organisation:

applies for publication on Cancer Council Victoria’s online listing of registered SunSmart members (SunSmart Listing) and Cancer Council Victoria (CCV) may approve or deny such application at its discretion;
consents to CCV publishing on the SunSmart Listing the Organisation’s name and logo, a link to the Organisation’s website, the year the Organisation joined SunSmart and the duration and due date for renewal of the Organisation’s membership; and
grants CCV a royalty-free licence to use the Organisation’s trade marks to the extent necessary to publish the Organisation’s information and logo on the SunSmart Listing.
